🚨 Injury Quick-Action Guide
1. Assess the Situation
- Life-Threatening?
- Campus: Call (541) 463-5555
- Off-Campus: Call 911
- Stable? If trained and willing, administer First Aid, and secure the immediate area.
2. Required Reporting (Within 24 Hours)
Identify the person involved to determine the correct paperwork:
If the person is... | Action Required |
|---|---|
| an Employee | |
| an Employee (Seeking Medical Care) | Complete Form 801 (in addition to the above form) |
| a Student or Visitor |
3. Prevention (The "Near-Miss")
See something? Say something! If you identify a hazard or experienced a "close call" where no one was hurt: Report a Safety Hazard
4. Supervisor Responsibilities
- Investigation: Must conduct a root-cause analysis within one business day. At LCC this is captured in part by the Employee Accident/Incident Form.
- Critical Incidents: For hospitalizations, amputations, or loss of an eye, call Risk Management immediately (541-463-5503). Note: We are legally required to report these to OR-OSHA within strict timeframes.

The Investigation Process
Supervisors are required to investigate all incidents to find facts, not fault. The goal is to identify the Root Cause using the Hierarchy of Controls:
- Elimination/Substitution: Can we remove the hazard entirely?
- Engineering: Can we install a physical barrier or ventilation?
- Administrative: Can we change the process or provide better training?
- PPE: What specialized gear is required as a final layer of protection?
