1. Assess the Situation

  • Stable? If trained and willing, administer First Aid, and secure the immediate area.

2. Required Reporting (Within 24 Hours)

3. Prevention (The "Near-Miss")

See something? Say something! If you identify a hazard or experienced a "close call" where no one was hurt: Report a Safety Hazard

4. Supervisor Responsibilities

  • Investigation: Must conduct a root-cause analysis within one business day. At LCC this is captured in part by the Employee Accident/Incident Form.
  • Critical Incidents: For hospitalizations, amputations, or loss of an eye, call Risk Management immediately (541-463-5503). Note: We are legally required to report these to OR-OSHA within strict timeframes.

You have the right to receive treatment from the health care provider of your choice, including:

  • Medical Doctors (MD) & Osteopathic Doctors (DO)
  • Physician Assistants & Authorized Nurse Practitioners
  • Chiropractors, Naturopaths, & Podiatrists
  • Oral Surgeons

Returning to Work

LCC is committed to your recovery through our Transitional (Light Duty) Work Program.

  • Medical Release: A written release from your physician is required before returning.
  • Modified Duty: If you have restrictions, HR will coordinate with your supervisor to identify temporary, meaningful tasks that fit your physical limitations.
  • Reinstatement Rights: Per ORS 659A.045-052, LCC protects your right to return to your position or a suitable alternative following a workplace injury.

The Investigation Process

Supervisors are required to investigate all incidents to find facts, not fault. The goal is to identify the Root Cause using the Hierarchy of Controls:

  1. Elimination/Substitution: Can we remove the hazard entirely?
  2. Engineering: Can we install a physical barrier or ventilation?
  3. Administrative: Can we change the process or provide better training?
  4. PPE: What specialized gear is required as a final layer of protection?
