Purpose

This procedure describes the process for accessing a patient's medical records. All medical records and information are confidential.

Narrative

Confidentiality

All medical records and information are confidential and are released only upon written permission of the patient, upon court order or as provided by law (Oregon Revised Statutes 192.525-530).

Patient Access to Records

Patients may review their own records.  To request access, submit a completed Authorization for Release of Medical Record Information to the Health Clinic.  Forms are available at the Health Clinic.
