Enrollment Services Document Intake Form

Hello and welcome to the Enrollment Services Digital Drop-box. We will be collecting your documents via this secure interface.

Please submit scanned or a high resolution photograph of your documentation to be uploaded. Blurry or illegible documents will be rejected and slow down the processing of your files.

This virtual dropbox will support the following file formats: jpg, gif, png, doc, docx, and pdf.

Please Note: If you are submitting a fillable pdf, be sure to save it as filled out, then verify that the pdf has the information you filled out. You can also print out the completed pdf, then take a photo and submit the photo of the completed form.
